My point was that leaders are severely unlikely to authorise the release of nuclear weapons. I forgot about submarine commanders, do they still have authority to launch their missiles without prior confirmation under certain circumstances (obviously excluding "war").

As far as I know, submarine commanders never had authorization to launch without orders from the chain of command.
The plan was you waited until the order came or you died first. Commanders were authorized, short of firing nuclear missiles, to do what they needed to survive.
