They're temporary files. Sounds stupid but that's it. Sometimes when a program's running it'll need to store more data than it'd be happy doing in memory, or some data that it'll only need to interact with another program. The program normally won't need that data beyond one reboot.

Depends. Some may well be in use by active programs, most will be forgotten about and can be culled. The general rule of thumb before things like the Drive Cleanup wizard was that if it was more than a week old, nuke it.

Delete old ones, they're unlikely to be needed. In fact you can probably get away with deleting everything up until the time you logged in today. The Drive Cleanup wizard in XP has long since stopped cleaning my Temp directory for reasons I cannot fathom, there's currently half a gig of shite in there that'll get culled.
Going by the 'Microsoft way', delete everything that is older than a week; that'll account for most of them.
