i'm using a visa card.
i filled out the jolt form and submitted it. i was then redirected to visa to make a password. it's now redirected me back to the jolt form only it's now blank. do i fill it out again and re-submitt or have you already taken my money?

scroll down to the bottom of that page, the form is blank but there'll be a confirmation your payment was sent.

To be perfectly sure just check your emails.
